Required Forms for Participating in Sporting Events

Click on the links below.   Please be sure that both parent/guardian and student sign in the space provided.  All forms must be filled out completely and be on file before student is allowed to practice or compete in competition.

*MEDICAL HISTORY form must be completed yearly.  This is completed by parent or guardian.

*PHYSICAL EXAM form must be filled out by a physician, physician assistant, chiropractor, or nurse practitioner .  Only students entering grades 6, 7, 9, and 11 must have a physical exam
